Dans Home Health Inc is a small, Medicare-certified agency on Santa Anita Avenue in Arcadia, serving patients across the San Gabriel Valley.

How we work

The work is straightforward: follow the doctor's orders, write down what happens in the home, and keep one team accountable for the patient.

Physician-directed care

Care begins when a doctor orders home health and signs a plan of care. Every visit a nurse, therapist, or aide makes follows that plan. We do not decide care on our own — we carry out what the physician has ordered.

Documentation back to the doctor

We document each visit and report changes — a wound that is not healing, a new fall, a blood pressure that climbed — back to the ordering physician. The doctor stays informed and adjusts the plan when the patient's condition changes.

One coordinated team

Nurses, physical and occupational therapists, a speech-language pathologist, a medical social worker, and home health aides work from the same plan of care. When more than one discipline visits a patient, they share notes rather than working in isolation.

Languages

Care depends on understanding what a patient is telling you. In a home health visit, the details matter — where the pain is, what changed since yesterday, which medication was missed.

Our clinicians and office staff speak English, Armenian, Russian, and Spanish. Patients explain symptoms in the language they think in.

When a patient can describe a symptom precisely in their own language, the clinician catches more and reports it accurately to the physician. Nothing is lost translating through a relative on the phone.

Our team

Home health care is delivered by six clinical roles. Each contributes a specific kind of skilled care, all working from the physician's plan.

Registered Nurse (RN)

A registered nurse manages wounds, medications, injections, and post-hospital recovery — and reports changes to your doctor.

Physical Therapist (PT)

A physical therapist works on strength, balance, and safe walking — using your own stairs, hallways, and bathroom.

Occupational Therapist (OT)

An occupational therapist retrains daily tasks like dressing, bathing, and cooking, and recommends practical home adaptations.

Speech-Language Pathologist (SLP)

A speech-language pathologist helps with speech, swallowing, and memory after a stroke or illness.

Medical Social Worker (MSW)

A medical social worker connects your family with community resources, counseling, and long-term planning.

Home Health Aide (HHA)

A certified aide assists with bathing, grooming, and personal care under the supervision of our nurses.
