Assessment scheduled
A nurse assessment is scheduled with the patient, and visit documentation flows back to your office.
What to include with a referral
Sending these items with the order lets us verify coverage and schedule the first visit without a follow-up call.
- Physician order signed by the referring physician.
- Patient demographics or face sheet.
- Insurance information.
- Relevant clinical notes and the current medication list.
- Face-to-face encounter documentation that Medicare requires.
Medicare eligibility reminders
For Medicare-covered home health, the patient generally needs to meet these criteria.
- Intermittent skilled need — nursing or PT/OT/SLP.
- Homebound status.
- Physician certification with the required face-to-face encounter.
