Every discipline below is delivered at home under a physician's order, coordinated by one local team.

Skilled Nursing

A registered nurse manages wounds, medications, injections, and post-hospital recovery — and reports changes to your doctor. Most often needed after a hospital stay, a new diagnosis, or a change in medications.

Physical Therapy

A physical therapist works on strength, balance, and safe walking — using your own stairs, hallways, and bathroom. Often needed after a fall, a joint replacement, or a long illness that left a patient weak.

Occupational Therapy

An occupational therapist retrains daily tasks like dressing, bathing, and cooking, and recommends practical home adaptations. Often needed when a patient can stand and move but struggles with the steps of everyday self-care.

Speech Therapy

A speech-language pathologist helps with speech, swallowing, and memory after a stroke or illness. Often needed when a patient has trouble being understood, swallowing safely, or following a conversation.

Medical Social Services

A medical social worker connects your family with community resources, counseling, and long-term planning. Often needed when a family is overwhelmed by the practical and emotional side of caring for someone at home.

Home Health Aide

A certified aide assists with bathing, grooming, and personal care under the supervision of our nurses. Often needed when a patient needs hands-on help with personal care alongside their skilled visits.
